Athletes in the Out-of-Competition (OOC) testing program use this form to assist USADA Doping Control Officers in locating athletes for doping control. Remember, athletes in the OOC program must complete and return an Athlete Location Form each quarter.

Athletes use this form to update USADA with temporary changes to location information, to ensure USADA Doping Control Officers have timely information to locate athletes for doping control. The USADA Change of Plan Form cannot be used in place of the Athlete Location Form.

This process provides a way for athletes to seek permission to use a medically necessary prohibited medication (except for glucocorticosteroids by inhalation or local injection and beta-2 agonists for asthma or exercise induced asthma, see ATUE). As an example, this TUE process would be used for any athlete requesting permission to use a stimulant for Attention Deficit Disorder (ADD). - 4 pages] | | | Abbreviated Therapeutic Use Exemption (Asthma Medications, Glucocorticosteroids)
Athletes seeking permission to use certain beta-2 agonists for asthma or glucocorticosteroids by inhalation and intra-articular or local injections must comply with this procedure. The Abbreviated TUE for the four prohibited beta-2 agonists that can be used with notification must be submitted prior to participation in sport. Athletes can complete this evaluation form to provide USADA feedback after any recent Out-of-Competition or In-Competition testing sessions they have experienced.
