Athlete Evaluation Form
This evaluation form is part of a comprehensive performance program that ensures USADA continues to meet the needs and expectations of all athletes subject to testing. Athletes can complete this evaluation form to provide USADA feedback after any recent Out-of-Competition or In-Competition testing sessions they have experienced.

TUE Forms

Therapeutic Use Exemption Form
In the situation that an athlete requires the use of a prohibited medication or substance for a medically warranted situation, that athlete should download, complete and submit a TUE form. Use of this medication or substance should only begin after hearing back from USADA or the athlete's International Federation (IF) that the TUE request was approved. The athlete should submit an application for a TUE no less than 21 days in advance of the time of testing.

EMERGENCY Therapeutic Use Exemption Form
This form should ONLY be filed in the case of a true medical emergency (e.g. an emergency room visits, etc.), and not simply because an athlete is filing a TUE late. Inappropriate use of this form may result in denial of the request, and the athlete will be asked to submit a basic TUE form in its place.

This document details the specific types of information that need to accompany the Therapeutic Use Exemption form if the athlete is requesting permission to use one of the four specific beta-2 agonist medications. Note: These medications are inhaled and are commonly used to treat asthma.

Abbreviated TUEs
Starting January 1, 2009, Abbreviated TUEs are no longer an option. Previously, Abbreviated TUEs were requested only for the use four types of beta-2 agonists (formoterol, salbutamol/albuterol, salmeterol, and terbutaline) and inhalation and local injection of glucocorticosteroids. Please see below for how the use of these substances will be governed beginning January 1, 2009.
